Formerly known as a Swing Bed, EBCH Transitional Care might be a good option for patients:
- who need a little extra time to heal and recover but no longer qualify for an acute hospital bed
- who want to be near family and friends closer to home
- who don’t yet feel confident managing their care when they get home
- who prefer assistance in a more casual care setting with home-like amenities
On-site services at EBCH Transitional Care help you recover with confidence and include, including:
- 24/7 rehab care by RNs and LPNs
- Physical therapy
- Occupational therapy
- Wound care and pain management
- Infusion therapies
- Cardiac monitoring
- Meals and nutrition counseling
- Social work
- Pharmacy
