Ely Community Pharmacy
Located by the east entrance shared by the clinic and hospital, the pharmacy offers prescription drugs, over the counter medications, competitive prices and ample, and accessible parking. Just call 365-8788 on Tuesday or Thursday morning before noon with your orders or refills (Ely only). Pay by check or credit card when your order arrives.

Rehabilitation
Please note: The Rehabilitation Department is moving to their newly remodeled space on the Main floor of the Hospital as of June 1. Please use the Conan Street entrance.
The Rehabilitation Department at Ely-Bloomenson Community Hospital and Nursing Home is a multidisciplinary team composed of Physical Therapy and Occupational Therapy services. An orthotist is also available on a monthly basis for bracing and prosthetic needs.
Our rehab staff services clients of all ages and a variety of diagnoses in outpatient, home health, hospital and nursing home settings. The staff at the rehab department consists of highly qualified professionals who stay current with the latest in treatment techniques by attending continuing education program regularly. They recognize each individual's uniqueness and develop personalized programs to meet their specific needs. Services are covered under Medicare and most insurance policies, with a physician's written referral. We believe hands-on treatment is an integral component to recovery from injury.

- Physical Therapy (PT) is the treatment and prevention of illness and injury by physical means to help people achieve their highest level of function following injury, illness, or surgery. PT helps people lead more active and independent lives. PT can help improve circulation, restore motion, relieve pain, strengthen muscle, prevent injury, improve balance, and promote healing. Goals are achieved through the use of stretching, exercise, physical modalities, manual therapy and soft tissue mobilization, and functional training techniques. Physical therapy treats a wide range of diagnosis including the following:
- Back injury
- Balance impairments
- Neck injury
- Sports injury
- Stroke
- Post-surgical (joint replacement, laminectomy, fusion, arthroscopy, etc.)
- Work injury
- Occupational Therapy (OT) treats a wide array of diagnoses. They specialize in treating upper extremity dysfunction including rotator cuff dysfunction, carpal tunnel syndrome, tennis elbow, golfers elbow, joint replacements, stroke, and pediatric developmental issues. Sports injuries and work injuries are all treated to maximize your return to play or work. The OT department will conduct personalized job site analysis, work hardening, ergonomic assessments and on-site educational seminars for groups or businesses.
Even if you haven't had one of the injuries listed above, you may be having trouble completing small tasks such as opening doors and jars, putting on your socks, etc. Our OT department can help you regain your independence with adaptive devices and education after a home assessment. We provide free home delivery and set up for rental and purchasing of many Durable Medical Equipment items. We offer our oxygen customers home service visits and 24 Hour emergency service. Ely Bloomenson Community Hospital Home Health Care provides skilled home care services within a 30 mile radius with 24 hour call availability. The goal of our staff is to provide services that promote quality of life by restoring, maintaining and facilitating health.
What is Home Health?
Home Health is a variety of services provided in your own home where you can obtain the medical care you need.
Am I eligible for Home Health Care?
Services are available to persons of all ages who are recovering from an acute illness or surgery, chronically or terminally ill and is temporarily unable to receive out-patient treatment.
How can I obtain these services?
You may be referred to Home Health Care by your physician, other community agencies, family, friends or yourself. Questions may be directed to the Home Care Office at 218-365-8722 or to your physician.
How will I receive help after 5:00 p.m.?
If you have an emergency dail 911. To contact the Home Health Nurse, call the hospital nurses station at 218-365-8710. Identify yourself as a Home Health Care patient and they will get in touch with a Home Health Care nurse.
What is the cost?
Charges for home Health Care services are covered by many private insurance policies, Medicare and Medicaid. Your physician must order the visits and your home health treatment plan must meet specific guidelines. You will be responsible for payment of services not covered by third party reimbursement. If you are covered by Medicare you must be homebound. This means you can only leave your home for physician visits, church, hair cuts or attend adult day care.
What services are provided?
Skilled nursing services are available by a Registered Nurse and/or a Home Health Care Aide. We will observe and evaluate your condition, carry out the plan of treatment and teach you about health care at home.
Home Health Aides provide personal care such as bathing, grooming simple dressing changes, feeding, routine exercises and other daily living activities. All care is directed and supervised by a Registered Nurse.
Physical Therapists will assist in developing a care plan and provide treatments ordered by your physician enabling you to regain lost or diminished physical abilities.
Occupational Therapists will assist in regaining independence of daily living.
